reporting safety concerns

Thank you for taking the time to provide feedback on safety concern(s).  This page was created as a means for an employee to confidentially submit a suggestion, voice concern regarding their safety or of those on the jobsite, and any other recommendation which may be discussed at the Safety Steering Committee. 

You can enter the jobsite so we can identify where the hazard applies or leave it blank.  The subject will be area of concern but could be left blank (personal protective equipment, trip hazards, fall exposures, supervision, etc.).  Lastly, the message box is an opportunity to provide detail of your concern.  As stated, this is an anonymous entry so the information you provide is up to you in helping FCI’s safety culture improve.  If your entry relates to an immediately dangerous to life and health concern, please report to your supervisor, safety director, or executive officer.
